Primary Care Physician

• Provide Direct Patient Care
• Diagnose, treat, and manage acute and chronic medical conditions in a primary care setting.
• Conduct routine physical exams, preventive screenings, and health risk assessments.
• Achieve and Monitor Quality Metrics (HEDIS/Stars)
• Align care delivery with established clinical quality guidelines (e.g., HEDIS, Medicare Stars).
• Ensure documentation and coding accuracy to reflect quality outcomes.
• Drive Patient Satisfaction and Engagement
• Foster strong physician-patient relationships through effective communication and personalized care.
• Address patient concerns promptly and follow up to improve overall satisfaction.
• Participate in Daily Team Huddles and Care Coordination
• Collaborate with interdisciplinary care teams during daily huddles to review high-risk patients and care plans.
• Coordinate with specialists, case managers, and support staff for holistic care delivery.
• Promote Preventive and Population Health Initiatives
• Identify care gaps and proactively engage patients in preventive care, such as immunizations and screenings.
• Use data-driven insights to support population health management strategies.

Requirements

• Requires an MD or DO.
• Current unrestricted medical licensure in applicable state(s) and board eligibility or board certification.
• DEA license required.
• Satisfactory completion of a Tuberculosis test is required for this position within the Carelon Health business unit.

Compensation: $232,000 - $282,785 per year & bonus eligible
